Job Description:
The Advanced Practice Provider is responsible for the evaluation
and treatment of patients with hematology and oncology conditions.
The Advanced Practice Provider works within the construct of a
multidisciplinary patient care team under the direction of a
physician. He or she works with additional clinical and support
staff to ensure safe, effective, quality person-centered care.
Job Duties:
- Provides all care in accordance with state and federal law as
well as PCI policies, standards of practice, and prescribed
workflows. In collaboration with a physician:
- Conducts patient interviews and physical evaluations to
evaluate and treat new and existing patients.
- Orders and/or performs laboratory tests.
- Order and/or performs diagnostic and/or therapeutic procedures
and testing.
- Collects and interprets diagnostic data.
- Diagnoses medical problems, develops care plans, and prescribes
treatments.
- Arranges for consultations with or referrals to other medical
professional or community services.
- Educates patient on health habits, self-examinations, behavior
management, need for follow-up assessments/treatments, at home
medical care, medication and other issues related to medical or
health problems.
- Utilizing available software, documents patient histories,
assessments, interventions, and interactions.
- Provides psycho-social and emotional support to patients and
care partners.
- Recognizes the roles of the interdisciplinary team, patient and
family care partners and utilizes them appropriately as resources
in planning patient care.
- Provides clinical information to the Revenue Cycle Management
Department as requested for the purposes of claims and
authorizations.
- Participates in prescribed research training exercises to
develop a proficient understanding of clinical trials.
- Applies knowledge gained to execute tasks and provide care as
prescribed by the trial.
- Communicates effectively with patients, care partners, members
of the healthcare team, and third-party agents or
organizations.
- Serves as a resource to colleagues and as a preceptor to new
employees as directed by organizational leadership.
- Utilizes resources in a responsible manner with a focus on cost
containment.
- Accepts personal responsibility for professional development
and growth.
- Participates in professional activities and organizations to
maintain knowledge of current trends, practices, and
developments.
- Respects and maintains confidential information.
- Demonstrates proficiency in HIPAA (Health Insurance Portability
and Accountability Act) concepts.
- Attends prescribed classes and trainings as required by the
organization and the clinical department.
- Participates in organizational committees and task forces.
- Performs related work as directed by the organization's
management team.
Minimum Qualifications:
1. Graduate of an accredited Nurse Practitioner or Physician's
Assistant program
2. Active certification as a Nurse Practitioner or Physician's
Assistant
3. Active NJ licensure as a Nurse Practitioner or Physician's
Assistant
4. Basic Life Support certification
5. Advanced Cardiac Life Support certification
